All masters programs students who are enrolled at McCormick Theological Seminary are required to complete a healthcare waiver of liability form.
In order to satisfy the Seminary's health insurance requirement and to register for classes, all masters program students who are enrolled at McCormick Theological Seminary are required to complete a healthcare waiver of liability form.
International students who do not maintain permanent resident in the United States must submit proof of insurance in addition to the waiver before arriving on campus or by June 15, whichever is sooner. Students participating in an intensive Hebrew/Greek courses in August are required to submit their waiver of liability by July 15. All other masters’ level students must submit their waiver by August 15.
The health insurance plan sponsored by the University of Chicago (United Healthcare)  is available to McCormick students and their dependents. Students may visit the UofC website to explore plan details. However, to enroll, contact Alicia Rhine in the Office of Student Affairs, firstname.lastname@example.org .
An alternative health option is available to McCormick students through EJ Smith, as long as there are no pre-existing medical conditions. Students who are interested in the insurance must self enroll. Visit EJ Smith’s website, www.ejsmith.com  for more information.
Under no circumstance is McCormick Theological Seminary responsible for medical bills incurred by students.
